  • In another case series of three post-surgical patients (one with a cervical fusion, another with lumbar discectomy, and one with lumbar laminectomy), chiropractic treatment (including spinal manipulation and mobilization) led to subjective improvements in pain and function — with only one patient noting mild, transient soreness, and no serious complications. 
  • A case report involving four patients with persistent low back pain after spinal cord stimulator implantation (a type of post-surgical scenario) showed that manipulation or mobilization was well tolerated. Two of these patients even had durable pain improvement and none reported new neurological symptoms. 
  • Another clinical case report described a veteran with failed back surgery syndrome (FBSS) who underwent chiropractic care (Cox flexion-distraction) and was able to reduce his reliance on long-term opioid therapy. 

2. Reviews & Theoretical Considerations

  • A narrative review of lumbar fusion surgery and its relevance to chiropractic practice found that although high-quality experimental studies are lacking, there are case reports of patients benefiting from flexion-distraction manipulation, manipulation under anesthesia, and other gentle chiropractic techniques after fusion. 
  • According to chiropractic-safety organizations, there is no strong epidemiological evidence that spinal manipulation increases the risk of acute disc herniation or other catastrophic complications in general, though care must be taken in post-surgical spines.

  1. Technique matters.
    • High-velocity, high-force “twisting” manipulations that directly involve the surgical (fused) levels are generally avoided
    • Instead, chiropractors use gentler techniques such as Activator, toggle-drop, or flexion-distraction
  2. Timing is critical.
    • Many sources recommend waiting months after surgery before initiating chiropractic care. Some suggest waiting as long as 6–12 months, especially after spinal fusion. 
  3. Patient selection and screening.
    • A chiropractor must be fully aware of your surgical history (which levels were fused or decompressed, what hardware was placed, bone healing status, etc.).
    • Imaging (X-rays or other studies) may be needed to assess how the spinal hardware is doing, and whether adjacent segments are safe to manipulate.
  4. Close coordination with your surgeon or spine specialist.
    • Before starting chiropractic care post-op, it’s wise to discuss with your spine surgeon. Some surgical hardware may impose mechanical limitations or contraindications.
